Attached is an alert we are issuing to the industry because we feel it is important to share information even though our investigation is ongoing.  The pipe system that failed had been regularly inspected at numerous monitoring locations. Our investigation is focused on understanding what happened and what we can do to prevent a similar incident.  Because safe operations are a core value at Chevron, we are conducting enhanced inspections of all components in similar pipe systems.
